It has not worked for me in Leopard at all. I think perhaps the corrupted 'docset' hypothesis is worth testing.
On another note, is there a doc set for the standard C and C++ libraries and STL? that would be really handy to have integrated.

Another thought...check the options at the top of your Documentation window. Where you see the "API" "Title" "Full-Text" toggles, and the others for doc sets, languages, and match rules across the window. Maybe something changed there? My defaults are...

Your bogus search results is worrisome. It would suggest to me a corrupted index. Maybe a bit radical, but maybe you could delete the docset and re-download it? I've never done it, but the docset is in /Developer/Documentation/DocSets. If you "Get Info" from within your Documentation window (through the gears menu at the bottom left) you can get more path information.
