When this happens to me, it is almost always because I accidentally clicked
the large button at the right end of an Xcode window's toolbar, and changed
that window from grouped to ungrouped or vice versa. It's easy to click this
button accidentally, especially when clicking a barely-visible corner of a
back window to bring it to the front, because the active area of the button
is larger than the button's image.
