This situation can definitely occur in Safari, as there's a WebKit
issue at the core of it. If there's any repeatable case, please do
complain, and we'll investigate and fix it. It's a very annoying bug
and we don't want people to have to live with it.
To our knowledge the bug itself is not sporadic; either the
triggering functionality is there or it is not. If this occurs on a
page and then goes away with a refresh, somewhere in the system the
data was likely cached. Once the issue is gone it should stay gone,
and we've updated all documentation that we believe exhibited this
issue.

I reported 100% reproducible (for me) steps in <rdar://problem/7199788>
1. Launch Safari
2. Empty Cache in Safari
3. Load <http://developer.apple.com/mac/library/documentation/Cocoa/Reference/Foundation/Classes/NSFileManager_Class/Reference/Reference.html

4. Click on "Low-Level File Management Programming Topics" near the
top next to "Companion guide"
5. Click on "File Management" under "Organization of This Document"
6. Click on "moveItemAtURL:toURL:error:" in the second sentence under
"Moving, Copying, and Deleting Files"
7. Scroll up to "Tasks" in the document (NSFileManager Class Reference)
8. Hover over "defaultManager" under "Creating a FileManager"
