What causes schemes to screw up and not show iOS Device or the Simulator?

I just checked out a fresh copy of my app that was touched by one of my coworkers.

The Simulator is now missing from the schemes.

Why does this happen?

For the life of me, I can not find out how to fix this.

Why does this happen in Xcode 4?  So many little things like this are such time wasters when compared to Xcode 3.x
