Depends on the RGB and the CMYK spaces involved, but each tends to include
some colors not present in the other, in most cases.

You don't; thats one of the limits of RGB drivers. But since images start as
RGB scans or digital photos, colors outside RGB are not going to be contained
in your images anyhow. In terms of CMYK defined spot colors, yes, you could
ask color recipes your printer can't reach... but then with an RGB driver you
are only approximating spot colors anyhow.

Sure, for vector and spot colors; for images the best deal for Hexachrome and
other six color systems is to use a large RGB space for your images, and
convert from there to Hex at the appropriate point in the process.

No, you can send the RGB directly to the RIP, and let it perform the
Hexachrome conversion for you, which is simpler, and does not require
complicated file formats.

HexImage and HexVector from Pantone come to mind, for those cases where you
actually want to seperate the files to six channels in advance.
