Is this the case in all instances? For example, If the ID color settings are set to preserve embedded profiles, and the embedded profile in an image differs from the default CMYK working space, when printing letting ID determine colors, there will be a conversion despite having "preserve CMYK numbers" checked.

If imported CMYK content has a profile embedded, and the CMYK color management policy is set to preserve embedded profiles, then that content is considered device independent and is immune to "preserve CMYK numbers".

Consider this check box functionally equivalent to temporarily changing the assumed source profile for all untagged CMYK content to match the printer profile you select in the print dialog.
