I've had similar reports from users of my apps which turned out to be just the mute switch. Depending on your audio category, your app will either respect or ignore your device mute switch. Users would have the mute switch on, hear audio out of iTunes but not from Thicket (my app) and figure that Thicket was broken, when, in fact, it was just respecting the mute switch. The fact that a restart seems to fix the issue would tend to indicate that this isn't the problem, but it might be worth a look anyway.
