If your mouse is in a window for App A, and you want to move it to
the menu bar, but all of the space between App A's windows and the
menu bar is filled by the windows of App B, then you won't be able
to get to App A's menus. As soon as you move the mouse off of App
A's windows, App B's menu bar will appear.
If the menu bars were attached to the apps' windows, and not fixed
at the top of the screen, then focus-follows-mouse would work.
