Ok, that sounds disturbingly familiar actually, does it generate any key events or events of any kind other than motion? If not this might be a case of the system not sending mouse events for the pen, or the events it is sending not being translated by X. Or actually, not sure how this is configured in XQuartz by default but it could be X isn't set up to understand button presses form a button number as high as is being used (less likely).
Along those lines though, is there a mouse compatibility option in the pen settings? Something like that exists in the other tablet environments I've worked in but unfortunately I don't have a good way to check on a mac.

OK, I tried xev to see what is going on.
Significantly, while the pen does generate loads of events, particularly motion notify events, it does not generate any button press or button release events when I tap the pen on the tablet (the usual way of indicating a click with the pen).
