It is working for me as expected.
used your commandline and I get blue letters on a white background
(this is with 10.5.5 and 2.3.1).
Note that I do not have any xterm defaults set in a XTerm or
Xresources local file.

Right. With -rv (or without any [-+]rv) you get the fg and bg
colors specified on the command line; with +rv they are swapped.
Expected behavior is that -rv swaps the fg and bg and +rv doesn't,
i.e. the opposite of what actually happens.
(And my friend acked the official bug report, so hopefully it won't
b e lost.)
