When you are saying to make a new custom executable.
As I am new with XCode : What does that means?
It means to use the "New Custom Executable" menu item in the Project
menu.
In my case it is a twain.ds I need to debug with current application
such as
Photoshop
1- Where do I tell in the project that the Lib must be loadable and
debug from a Target directory?

2- Where do I tell in the project that the executable for the debug
session will be...?
In the inspector for the new custom executable.
