In your keychain, you shouldn't see any "Apple Worldwide Developer Relations Certification Authority" certificate and no "iPhone Developer: <first name> <last name> (ID)" certificate either.
In your project, when looking at build settings of the project's info, in the "Code Signing Identity", there shouldn't be an entry with "iPhone Developer: <your first name> <your last name>". I helped my manager that was having a similar issue and that's what we did, clean up everything and started from scratch, recreating provisioning profiles and downloading his Apple certificate and the newly recreated provisioning profiles and it did work.
