You might take a look at rdar 7033595 (closed, evidently as "behaves properly"), where I supplied screenshots and a full explanation. The gist of it is: in a typical session with the docs, I looked at only 2 documents, and I needed to go back and forth between them because they are closely related. The Back popup contained 6 items that I didn't even look at; they appeared while I was typing "NSString" in the search field. The next 20 items are identical: they all say "NSText Class Reference." They are links to different places in that doc, but you can't tell what they link to.
XCode 2.0 had a History list that showed the recent documentation files. That was removed in XCode 3. We still have it in the source code viewer, fortunately, but I really miss it in the doc viewer.
