Did you try to drag the file from the Finder into the image well in Icon Composer corresponding to the size you want?
I had a nasty conversion when I was trying to use copy/paste from PS, then I read somebody recommending dragging the file from the Finder into the image well in Icon Composer. That took care of the problem...
