I'd say it's organizational/managerial.
I suspect the Xcode team simply has too few resources available to adequately tackle their given tasks while maintaining high quality.
At a bare minimum they need to add support for new versions of iOS and OS X every year (the Xcode team must have been over the moon when they heard about the decision to put OS X on a yearly release schedule too). Plus I assume they have long-term efforts on the go to add support for improvements in runtimes/languages (or even completely new ones) and it's easy to see how only the most central use-cases and high priority issues get significant attention.
I'm not trying to excuse Apple. Xcode's quality is lacking and this needs to be addressed by management.
Apple has undermined developer confidence in a number of ways over the last couple of years and next to the shoddy handling of the ongoing sandboxing saga on OS X, Xcode 4 is the #1 source of frustration with Apple amongst the iOS and OS X devs I know over here in Europe.
Developers can't understand how the world's richest company (as has been mentioned here before) can release something that fails to meet their most basic requirements of stability and performance.
I bet the bright and talented engineers working on Xcode care deeply about the product, but from where we're sitting it looks like Apple as a whole doesn't place a high priority on the quality of their developer tools.
