Ugh, you're right. I tried again, this time running XQuartz, and now my qemu session successfully opens the X window (and this is with the original X11.bin, I backed out X11.bin-PR-423). So, my guess is that my problem was due to some mismatch between X11.app and things in /opt/X11, which come from XQuartz. For example, my $PATH searches /opt/X11/bin before /usr/X11/bin. But its working now, so I'm not going to worry about it.
But when I run XQuartz, I've lost my X11 application menu. Is there an easy way to copy over that info from X11 to XQuartz? And where is it stored? I ran "defaults read > defaults.all", and when I search the output, I have a com.apple.x11 with apps_menu, and an org.x.X11 with apps_menu, but they both have a long list and I'm only seeing the defaults in the "Applications" menu when running XQuartz. I used to run XQuartz long ago, and the org.x.X11 stuff might be left over from that, has the format changed?
