I see this primarily which some other process grabs focus, often
iTunes when I plug in my iPhone or iPad.
Another place I've had another process grab focus is when I'm in
an xterm and have an old VMware X11 console running and booting a
virtual machine. The VMware console process with grab focus from
another X11 process.
