In fact, there's a semi-recent thread on the main x.org list about
this headache, and they're trying to put together a posix.cf config
for xmkmf that should work on all modern systems, so they can nuke the
whole repository of config files... I think it was last week. Start
there if you want to dig the xorg mailing lists.

This. The Apple developers inform us that upstream xmkmf is broken,
and dead at X.org.
Well, we're not just making it up - go ahead and ask them. :-)
But out here in the trenches, I still see it on my fresh-as-daisies
Ubuntu Hardy Heron install. Even if I completely
uninstall and reinstall all things X, xmkmf comes back with the
rest of the dev tools. And it even works, at least well enough to
build
the few things I've built manually.
I'm sure there are a number of Linux systems that continue to ship
the bits themselves because it's just as easy to do so as not.
Where the Linux folks have a distinct advantage, however, is in the
number of people willing to keep those bits current and relevant.
We, on the other hand, have one Intern working part-time and n
volunteers, where n may now be zero - I haven't seen anything
recently to suggest otherwise.
As folks have probably noticed, I've also tried to throw additional
money at the problem without success. - if I had a guitar, this is
the part where I'd start singing "Where have all the contractors
gone? (long time passing...)"
